Day Camp Registration Form

You can register 1 or 2 campers with this form, provided that they have the same Parent/Guardian Information.

All information marked with a “*” is required. The form will NOT submit if the required information is not filled in.

If registering a single Camper, you can skip Camper 2 by clicking on the Parent/Guardian tab.

You must Click/Tap the SUBMIT button on the PAYMENT page after making your payment, for the form to be sent to us. You will receive an email confirming the form submission.
