Day Camp Registration Form
Before registering you need to download and read the Liability Waiver Document using the button below.
You can register one or two campers using this form provided that the Parent/Guardian information is exactly the same for each camper.
If you want to register more than two campers from the same family, please contact the office as there is a discount for three or more campers from the same family.
Note: All fields marked with a “*” symbol are required. The form won’t submit without this information.
Payment Requirements
You must pay by credit/debit card for at least one week/per camper when submitting your application.
Please note full payment is required for campers to attend camp.
If you are registering for multiple weeks, the additional weeks must be paid in full by May 15.
Payment can be made by Visa or Mastercard credit/debit card when registering online.
The program is administered through the Smiths Falls Curling & Squash Club and your payment will be to them.
We uses Zeffy as our secure payment processor.
Zeffy is unique in that they don’t charge Not or Profits any processing fees for credit/debit card payments.
IMPORTANT
Zeffy offers an OPTIONAL way for you to support their work with Not For Profits by adding a percentage donation at Checkout.
This is completely OPTIONAL and you can select “OTHER” from the drop down menu at Checkout.
